Traffic Division Appearance Plea of Guilty and Waiver |
Please Read Carefully
This form is furnished for your convenience in the event you have been charged with an offense which does not require your appearing in person before the court an you desire to plead "GUILTY" and pay the prescribed fine and costs.
Unless the front side of the ticket you received is marked "MUST APPEAR IN COURT" you may enter a plea of "GUILTY" by mailing or delivering to the office of the Clerk of the Circuit Court, Sangamon County Courthouse, Springfield, Illinois, your signed plea of "GUILTY" and pay the prescribed fine and costs. Your plea of guilty and payment of fine and costs must reach the clerk's office no later than 5 days before the date the officer has shown for you to report to the court. If you elect to deliver our plea of guilty, you should wait at least 10 days after the date the ticket was issued.
Supreme Court Rules provide for the following fines and costs to be paid on pleas of guilty to offenses not requiring court appearances:

If cash bail was posted, it will be applied to payment of fine and costs. If a driver's license or bond certificate was deposited, you must pay the full amount of the fine and costs before the license or bond certificate can be returned. Payment must be in the form of money order, cashier's check, or travelers' checks issued by major banks or express companies.
