Protect Your Identity
You are NOT required to submit your Social
Security Number for court documents. If a disclosure
of a Social Security Number is required, only the last four
digits of the number will be used.
Pursuant to Illinois Supreme Court new
Rule 138, effective January 1, 2012: Unless otherwise
required by law or ordered by the court, parties shall not
include Social Security Numbers in documents filed with the
court, including exhibits thereto, whether filed
electronically or on paper.
